Family Cheering

We are committed to developing and implementing strategies that will support participation and healthy active lifestyles for older adults and all citizens in Perth County in keeping with the World Health Organizations' eight Age-Friendly Community Dimensions.

Perth County Age-Friendly Community Action Plan 

In the fall of 2015, the County of Perth was awarded an Age-Friendly Community Planning Grant by the Government of Ontario, to develop a plan that will make Perth County more supportive to its senior residents as they age. The following documents lay the groundwork for the next steps, which will be the development of implementation plans based on the actions identified:

What is an age-friendly community?

 The Age-Friendly Cities initiative was launched by the World Health Organization (WHO) in 2006 to enhance the health, participation and security of older adults. The WHO established eight dimensions related to creating age-friendly communities:
  • Outdoor Spaces & Buildings

  • Transportation

  • Housing

  • Respect & Social Inclusion

  • Social Participation

  • Civic Engagement and Employment

  • Community Support and Health Services

  • Communication and Information

 

Next Steps

In order to develop implementation strategies, a new committee will need to be formed, which will include staff from the County, the Member Municipalities, the City of Stratford and the Town of St. Marys, as well as organizations who work with seniors, and seniors from across the County.

The South West Self Management Program
The South West Self Management Program at the South West Local Health Integration Network (LHIN) supports people living with chronic disease, their caregivers and also health professionals. It offers free workshops and resources across the region to help support people in making healthy lifestyle changes, giving them the tools, knowledge and skills to be successful. People can register for workshops online or they can call 1-855-463-5692 (toll free).
